The Feedback Form is for both compliments and concerns you may have in regards to working with me. At anytime you wish to lodge this form you are welcome to do so.
The Complaint Form can be used either for lodging a Complaint about myself and the services I provide to you, or it can be about another area of your life. All information is confidential.
Any complaints about CS SC are viewed as a way for me to learn and improve upon services.
The Feedback and Complaint Forms are in Word Documents, so you can open them up & type in your answers.

I have also attached information from the NDIS Quality and Safeguards Commission on how to make a Complaint with them on your rights to make a complaint and how to do this.
A big part of Christine Stammers Support Coordination or CS SC, is Feedback from people to let me know of what's working well, what's not and how I could improve upon my services.
The only way anyone or any service can grow or improve is through having an open approach, where Participants and their family members feel able to communicate with me areas of concern , or difficulties.
When a Participant (and their main decision maker) enters a Service Agreement with me, part of this is having regular Feedback reviews, which is part of the NDIS Quality and Safeguards, Standards - To ensure I am working within the guidelines of these standards but more importantly, we are working as a team.
Where you are in the "driving seat" of your decisions; you are treated with dignity and respect across all areas and services under your NDIS Plan funding; and by the end of the Support Hours under your plan, you no longer need my services.

The only time that this confidentiality would be broken is if you were at risk of harming yourself, or others, or you yourself were at risk of being harmed through Abuse, Neglect or Exploitation. This is line with the National Disability Standards and HSQF Standards.
